{"id":334,"date":"2016-08-14T15:37:22","date_gmt":"2016-08-14T15:37:22","guid":{"rendered":"http:\/\/www.prolocodeltrivice.it\/?p=334"},"modified":"2016-08-14T15:37:22","modified_gmt":"2016-08-14T15:37:22","slug":"cure-termali-ottobre-2016","status":"publish","type":"post","link":"https:\/\/prolococittadicaserta.it\/?p=334","title":{"rendered":"Cure termali &#8211; Ottobre 2016"},"content":{"rendered":"<p style=\"text-align: center;\"><strong><u>A V V I S O<\/u><\/strong><\/p>\n<p>La <strong>Pro Loco del Trivice<\/strong> in collaborazione con le <strong>Terme di Telese <\/strong>organizza soggiorni pendolari per <strong>cure termali<\/strong> con<strong> trasporto gratuito <\/strong>in pullman presso le <strong>Terme <\/strong><em>di <\/em><strong>Telese. <\/strong><\/p>\n<p><strong>I<\/strong>l periodo di cura sar\u00e0 articolato su <strong>12<\/strong> giorni consecutivi, <u>escluse le domeniche<\/u>,\u00a0 da <strong><span style=\"color: #ff0000;\">luned\u00ec 17 Ottobre<\/span> <\/strong>a <span style=\"color: #ff0000;\"><strong>sabato<\/strong> <strong>29 Ottobre 2016<\/strong><\/span>.<\/p>\n<p>L\u2019impegno durer\u00e0, prevedibilmente, dalle ore <strong>08:30<\/strong> (partenza) alle ore <strong>12:30<\/strong> (rientro)<\/p>\n<p><strong>Le cure convenzionate con il S.S.N.<\/strong> sono:<\/p>\n<p><strong>fanghi e bagni \u2013 idromassaggi \u2013 cure ginecologiche \u2013 bagni caldi \u2013 cure inalatorie ventilazioni polmonari e insufflazioni endo-timpaniche.<\/strong><\/p>\n<p>Le <strong><u>domande di adesione<\/u><\/strong>\u00a0 potranno essere presentate presso la ns. sede <strong><u>da subito<\/u>\u00a0<\/strong>e, <strong><u>come termine massimo<\/u><\/strong>, entro il<span style=\"color: #ff0000;\"><strong> <u>30 Settembre 2016<\/u><\/strong><\/span>.<\/p>\n<p>Saranno prese in considerazione solo le domande <strong><u>complete<\/u><\/strong> della seguente documentazione:<\/p>\n<ol>\n<li><b><strong>ricetta medica in originale del medico di famiglia <\/strong>con la prescrizione del ciclo di cura e relativa diagnosi;<\/b><\/li>\n<li><b><strong>copia del documento di riconoscimento;<\/strong><\/b><\/li>\n<li><strong>copia della tessera sanitaria. <\/strong><\/li>\n<\/ol>\n<p><strong>La realizzazione dell\u2019iniziativa <u>\u00e8 subordinata<\/u> al raggiungimento del numero minimo di almeno <\/strong><strong>50<\/strong> <strong>(cinquanta) partecipanti per ogni pullman.<\/strong>Eventuali ed ulteriori informazioni possono essere acquisite presso la Pro loco (<em>\u00e8 disponibile anche materiale illustrativo<\/em>) nelle giornate del <strong>luned\u00ec \u2013 mercoledi\u2019 e venerdi<\/strong> (dalle ore <strong>16:00<\/strong> alle ore <strong>19:00<\/strong>) ovvero, considerando il <strong>periodo delle ferie estive<\/strong>, telefonando al numero\u00a0<strong>345 2622202.<\/strong><\/p>\n<p><a href=\"http:\/\/www.prolocodeltrivice.it\/wp-content\/uploads\/2016\/08\/Modulo-iscrizione-terme.pdf\">Modulo iscrizione terme<\/a> (scarica il modulo)<\/p>\n<p style=\"text-align: right;\"><strong>IL PRESIDENTE<br \/>\n<\/strong><em>Giuseppe IANNIELLO<\/em><\/p>\n","protected":false},"excerpt":{"rendered":"<p>A V V I S O La Pro Loco del Trivice in collaborazione con le Terme di Telese organizza soggiorni pendolari per cure termali con trasporto gratuito in pullman presso le Terme di Telese.
